First Visit Checklist
Please make sure you bring all of the following information with you on your first visit to The Zangmeister Cancer Center:
- Patient information form
- New Patient History Questionnaire
- CD and/or report of past radiology exams (if available)
- Current insurance card(s)
- Pharmacy card
- Driver’s license or photo ID
- Co-payment for first visit
- Hereditary Cancer Questionnaire (optional)
